• You must be 16 years old or over (you can't sign up online if you are underBe 16 years old or registering your child). Click here for under 16s sign up info .over
• You must beBe a New Zealand citizen/resident (an eligibility check may be needed for people not born in New Zealand).NZ)
• You must haveHave a unique, personal email address (NOT a shared email address). You can only register one SmartHealth account per email address.address)
• You will need to provideProvide one form of photo identification plus one current clear photo of your head and shoulders (selfie).
