Registering online for SmartHealth

• You must be 16 years old or over (you can't sign up online if you are under 16 or registering your child). Click here for under 16s sign up info .
• You must be a New Zealand citizen/resident (an eligibility check may be needed for people not born in NZ).New Zealand).
• You must have a personal email address (NOT a shared email address). You can only register one SmartHealth account per email address.
• You will need to provide one form of photo identification and take aplus one current clear photo of yourself (headyour head and shoulders selfie).(selfie).
• Scroll down and click on Fill in e-Form to register online.
